Acute Postinfectious (post streptococcal) glomerulonephritis

Etiological agents

1. Bacteria -->Streptococcus (group A,B-hemolytic),Staphylococcus

2. viral-->measles,hepatitis B,hepatitis C

Pathogenesis
Following infections to etiologic agents


formation of immune complex (complement and IgG formed against the insulting agents – in circulation or in-situ)


stimulate further release of Growth Factors and Interleukin


stimulate proliferation of glomerular cells (mesangial cell and endothelial cells)


glomerular cells secrete nephrotoxins (oxidants, protease)


activation of leukocytes (primarily neutrophils)


leukocytes infiltration within glomeruli


capillary walls damage (due to inflammatory insults)

Morphology

- increased cellularity of glomerular cells due to:
  • proliferation and swelling of endothelial and mesangial cells
  • infiltration of leukocytes (neutrophils & monocytes)

-necrosis of capillary walls

-crescents within urinary space

1) utk pathophysiology bg APSGN ni, just citer pasal hypersensitivity type 3 je.. dah cukup..

2) cresentric shape tu biasanya dah masuk rapidly progressive glomerulonephritis..
