how does hepatitis cause increment in both direct and indirect bilirubin?

Perlu tahu ke mechanism dia..huhu, nak try jawab ek.. kalau salah sila bagitau. Maklumlah saya budak baru belajar.
Ok, firstly kena tau yg hepatitis tu kan inflammation of the liver. So bila nama dia inflammation ni kan, mcm2 mana pon mesti libatkan destruction of hepatocytes. So bile destruction of hepatocytes berlaku, aka ada dua benda berlaku..
1) Of course la reduce bilangan no of hepatocyte kan.. so akan reduce bilangan hepatocyte yg available utk amik unconjugated bilirubbin kat dalam blood.. Betoi tak.. Tapi mesti korang tertanya2 drpd mana datangnya excess unconjugated bilirubbin lak sedangkan xde hemolysis blood. So kat sini ada benda seterusnya yg berlaku..huhuhu
2) Korang kena ingat yg source bilirubbin tu 70-80% drpd destruction of blood. 10% drpd destruction new blood drpd bone marrow.. so lg 10% drpd mana.. so korang ingat la destruction of hepatocyte akan hasilkan benda2 ni; Intravascular hemolysis, turnover of other haem protein kt dlm liver mcm cytochrome dan last skali free haem pool kat dlm liver.. Benda2 ni akan dihantar ke reticuloendothelial system utk ditukarkan kpd unconjugated bilirubbin…so drpd sini la benda Allah tu datang.

So akibatnya ada dua benda berlaku…
1) Conjugation still bleh berlaku sebab ada lg hepatocyte yg available (single ni, leh msk meminang..huhu gurau jek
2) Akan tetapi ada excess unconjugated billirubin kesan drpd liver destruction tu + x cukup hepatocyte maka menjadilah mereka uconjugated billirubin.

So sebab tu lah kalau problem kat liver je mesti ada unconjugated+ conjugated billirubin. Kan kan kan???? Huah huah huah.. Mr Jack yg poyo..

hasil diskusi saya:

1) hepatitis > inflammation > hepatocytes injured > less capacity to conjugate the unconjugated bilirubin > excess unconjugated bilirubin

2) kalo inflammation > hepatocytes swelling > obstruct intrahepatic bile duct > conjugated can not go to intestine > soluble in blood > excess conjugated bilirubin

excess conjugated billirubin juga disebabkab failure of bilirubin secretion in bile acid.. sebab ada impairment pada energy-dependent steps..
